Patient Chart
Everything you need to know about a patient — in one place.
Open a patient chart
Go to Patients and click on any patient card. The chart opens with four tabs at the top: Overview, Records, Prescriptions, and Vitals & Labs.
The patient's name, Health ID, and basic demographics are shown at the top of the chart on all tabs.
Overview tab
Shows the patient's basic information — date of birth, gender, country, phone — plus a summary of their most recent prescriptions. Use this tab to get a quick picture before a consultation.
Records tab
Shows all documents the patient has shared with you, plus documents you have uploaded to their chart. You can upload new files directly here. Use Query Records to search this patient's documents with a natural language question.
Documents you upload here are visible to the patient in their own Records page under "Provider Shared".
Prescriptions tab
Lists every SOAP note and prescription you have generated for this patient, in reverse chronological order. Click any entry to view the full note, or download the formatted prescription PDF.
Prescriptions are automatically stored when you save a note from the Ambient Scribe. You can also add notes manually from this tab.
Vitals & Labs tab
Shows the patient's key vitals (blood group, weight, height, age) and a chronological timeline of past consultations. Each timeline entry links to the scribe session where the note was created.
The consultation timeline is derived from appointments. Vitals are populated from the patient's profile and records.
AI Record Query
From the Records tab, click Query Records to ask natural language questions about this patient's documents. For example: "What medications has this patient been prescribed in the last 6 months?"
